Summer Camp Forms/Paperwork

Print out the following forms using Adobe Acrobat.

1. Summer Camp Registration form
   
2. Financial Aid Application. Complete and send this form only if financial aid is needed.
3. Camp Health Form. Health History must be completed every year. Back of form must be signed by physician or nurse practitioner and physical date must be within 2 years of camp session.
4. Epi-Pen/Asthma Inhaler Permission Form. If your daughter needs to carry an epi-pen or asthma inhaler with her at all times while at camp, then this form must be completed. Complete this form and bring with you to camp.

Directions - for our girl's safety, we do not publish directions to our camp on the website. Please refer to your parent handbook for directions. If you have misplaced the handbook, request another copy from info@girlscoutsgwm.org.

Summer Camp Registration Information

• Register Online - Online registration is not available for resident camp Leadership programs and resident camp family weekend programs.
• Register by paper - Complete the registration form and return to Girl Scouts of the Green and White Mountains with a non-refundable deposit of $50 per program

Please Note:
Registration is ongoing until programs are full. Online registraton will close 2 days before a program's start date.

• All girls are welcome. If you are not currently a registered Girl Scout, you will be prompted to include a membership fee of $12 (this fee is for national membership in Girl Scouts of the USA) Please review the Girl Scout Promise and Law.
• Bring a friend incentive - The deadline for this program has passed.
  Bring a new friend to camp and earn a special incentive! If you recruit a new camper, who has not attended one of our summer programs inprevious summers, you and your friend will receive a $20 voucher to use at the camp trading post! to be eligible, you complete our online form by June 1, 2010 and provide complete information about both campers. Each camper is eligible for one voucher only. If the camp does not have a trading post, the voucher can be applied to a list of items available at the beginning of the summer. Campers mya not be siblings.
• Buddies - once you have registered, you will receive a Camper Information form on which you can request a buddy. Only one buddy is allowed per person. Buddies must attend the same session, unit and be in the same age bracket. Placements are not guaranteed and will be made at camp by the director.
• Change requests will be accepted after March 11th. there will be a $10 rebooking fee for changing programs once a girl's registration has been processed. This fee does not apply if the council has to move campers, if a program is cancelled or if a cmper moves from a wait list to the program of her choice
• To pay with cookie dough, subtract the estimated amount of cookie dough from the total due. Once you recieve your Cookie Dough certificate, it can be mailed with final payment.
• Resident Camp Family Weekend Programs - Complete a summer camp registration form and return to GSGWM with nonrefundable deposit.
